Free feedback form generator
A feedback form gives users a place to complain and suggest that is not a public review. Because it is on your own site and posts straight to your inbox, you hear about bugs before they become public issues.
Unlike a survey tool, the form is plain HTML you control: one page, one form, no branding, no scripts.
Fields to include
A rating plus free text is the pair that works: the number tells you if the user is happy, the text tells you why. Keep name and email optional -- anonymous feedback is still feedback, and forced identities get fake ones.

Spam tips
- The hidden _gotcha field is your spam filter. Public feedback pages are a favourite target for link-spam bots; a filled honeypot means the post is stored as spam and never emailed to you.
- Keep the rating as a number the visitor types, not a widget -- it works without JavaScript and posts cleanly.
- Do not ask more than four questions. Long feedback forms are abandoned halfway.
